Can I use the evaluation forms with my own name and logo?
Yes, you can add all your own practice details to the form. They're all yours.
Will I have help understanding the templates?

Each resource comes with a step-by-step video tutorial showing you how to download, edit and use each template.

Each resource also has two examples. A designed example and a completed patient example.
Can I modify the templates?
Yes.
Each evaluation resource has:
1) A Canva version that is completely changeable to be modified in wording and design.

2) A Word Doc version that is editable.
​
3) A PDF version that can be sent to patients or completed on your phone or tablet.
For how long will I have access to the templates?
All templates are downloadable. As long as you don't delete them, you'll always have them.
What if I accidentally delete one of the templates?

What makes these templates different
from everything else available online?
1: Most online protocols are not in a usable format.
You have to sit and take the time to copy and paste each test into a document and spend hours putting these evaluation sheets together yourself. We’ve done all that work for you so you can print and use them within your practice within minutes of downloading them, saving you time, effort, energy and resources.
2: Most online protocols can’t be edited to suit the needs of your practice and patients.
We’ve fixed that by making multiple versions of each template so that there are several ways for each resource to be edited, modified, adjusted, branded, customised, designed and other similar synonyms.
3: Most online forms don’t allow adding your practice name, contact details, logo, or branding colours and design.
This means it looks less professional when shared with patients, colleagues, or referring practitioners. So we’ve given you Canva templates to copy and add all your logos, colours and designs to make them fit your professional image.
Not sure what to design the templates? No problem; each template has a designed example from which you can draw inspiration.
4: Most online evaluation forms only allow space for one attempt at passing each test.
Another big problem we’ve tackled is that patients hardly ever pass the entire battery of end-phase tests on the first attempt. So, we’ve provided space for multiple attempts to complete each test. A massive advantage to this is that both you and the patients can also see how they fared on their previous attempts to pass the test to monitor their progress. This means that even if they don’t pass - you can still show them their improvements right there and then without having to search through tons of older documents to find their previous results.
5: Most online forms either have to be printed or completed online.
We’ve given you the flexibility of both. Want to eliminate clutters, mountains of paperwork and thick files with too many pages of notes?
We’ve made editable PDF versions of each template. This means you can open the PDF and fill in the values directly into the PDF, save it and immediately email it to the patient or referring practitioners.
These editable PDFs also allow you to fill out your practice name, and add contact information and a logo to always appear professional.
Don’t have Wi-Fi, electricity, or battery, or do you just prefer good ‘ol pen and paper? We’ve included unbranded, grey and white versions to make them as print-friendly as possible.
6: Most online forms only give you space to fill in the results without any notes.
This means you end up scribbling all your notes in the margins. We’ve allowed space for you to rate the patient’s RPE and pain for each test. For example, if a patient can only do 12 hamstring bridges and then stops due to pain - you can note that they stopped due to pain instead of weakness. When you test them again a few weeks later, you can note the improvement, plus that it was pain-free this time.
7: Most online forms don’t allow space for specific patient details.
We’ve included two other elements that often bother practitioners: remembering whether the patient had injured their left or right side. When you see a bunch of patients a day, it becomes hard to remember if it is the left or right side for each and every patient unless you write it down. That automatically stops that awkward moment where you’re palpating the patient, and they wonder why you’re not even touching their injured knee.
We’ve included their operation or injury date on each form because this helps put things into perspective when they are battling to reach certain rehab milestones. For example, you can remind them that they’re only 12 weeks post-op, and this progress is entirely normal.
8: Most evaluation forms will force you to use a specific metric.
We’ve given you the space to use centimetres or inches or any measuring metric you choose. That way, you are not forced to do annoying conversions just to make the form work.
9: You’re not always sure what research other online forms are based on.
Another massive advantage to these templates is that you don’t have to spend hours and hours reading hundreds of studies to find what you should be testing and what the patient should be achieving, as these templates are already based on the latest high-quality ACL rehab evidence-based protocols.
