I was originally going to name this post:



“My ACL don’t jiggle, jiggle - it holds”, which I thought was hilarious and it took every inch of my self-control not to.



Today’s questions are:


  • How to know when your athlete is ready to return to running after ACL reconstruction?


  • If they are, what would a proven evidence-based running program look like?




Let’s look at a study that came out in June 2022.



Pairot de Fontenay and colleagues tested a novel return to running algorithm on 34 athletes. 



Spoiler: It worked like a charm.



97% was able to complete it successfully with only one subject having to stop due to pain.



It was a 2 week program that was progressed based on the following algorithm.



But before you even get to the progressive program, you need to know if your athlete is actually ready to start running.



The researchers used 5 tests to evaluate the patients:



1) The IKDC Subjective Knee Form 



2) The ACL-RSI Scale



3) Isometric Quadriceps and Hamstrings Strength



4) Step-Down Endurance Test



5) The mSEBT




Here’s the crazy part:



Only ONE of these tests were actually able to predict if the patient would have a successful return to running. 



In fact, if you could pass this one test, you were 3.11 times more likely to return to running without an exacerbation of symptoms.
